To submit a request for copies of vital records, print out the appropriate form, sign and return to our office with the appropriate fees. These documents are in "PDF" format. You can type and then print them, or print and fill them out by hand.

| All records are open to the public, with the exception of birth records for which we can only verify information. In the case of a deceased ancestor, this record may be purchased. |

Fee for Genealogy Services
|
| For genealogy purposes there is a charge of $5.00 per day, which gives access to the marriage and death records. Birth records of deceased individuals are available to heirs however you must purchase a certified copy of the record. |
