CONTINUUM OF CARE- HIV CASE MANAGEMENT

The Continuum of Care Program is a community based program that offers a broad spectrum of case management and support services to people living with HIV/AIDS in the Upper Peninsula (U.P.) The goals of the program are to reduce barriers to ensure that a person living with HIV/AIDS has access to medical care and medications, and can maintain medical care and remain on medications
The Continuum of Care is administered by the Marquette County Health Department, but serves all fifteen (15) counties of the U.P. Funding is provided through the Michigan Department of Health and Human Services, Ryan White HIV/AIDS Treatment Modernization Act (RW), and the Housing Opportunities for People with AIDS (HOPWA) and the Tobacco Use Reduction in People Living with HIV project (TURP)
All information about clients living with HIV/AIDS is kept strictly confidential. Staff takes great care to ensure client’s privacy and confidentiality
Financial assistance is based on income and availability of program funds
The COC Program must be payer of last resort
Program staff includes a multi-disciplinary team approach of both a social worker and nurse
Eligibility

Residents of the U.P. who are living with HIV

Referrals can be self-directed or made by other health care providers with the written consent of the client.

Income must be at or below 500% of current federal poverty levels
Services

Medical Case Management- help with managing medical appointments, lab work, diagnostic test or medication. COC nurse of often present at medical appointments with Infectious Disease provider.

Assistance with applying for appropriate insurance programs such as Medicaid, Medicare (ABD), and Affordable Care Act plans

Referral to other resources

Assistance with applying for other resources

Ongoing assistance and support for maintaining appropriate benefits and services.

Mileage Reimbursement for HIV related medical appointments

Possible financial assistance for unpaid HIV related office visits or lab work.

Possibly rent/utility assistance

Tobacco cessation counseling, support and education
