RABIES SURVEILLANCE AND BITE REPORTING
If you believe you may have been bitten scratched by an animal or may have been exposed to a bat, please contact (906) 475-4195, or complete the following Animal Bite Report Form and submit by fax to (906) 475-6500.
If you believe you have been potentially exposed to rabies from a source other than a bite such as a scratch or been in a residence with a bat, please complete the general rabies complaint form and submit by fax to (906)475-6500 or call (906)475-4195 for further assistance.
