Gwinn Adolescent Health Clinic

The Gwinn Adolescent Health Clinic provides a place for students to access health care right within the school building. A Nurse Practitioner, under the direction of a licensed physician, will be available to students a minimum of 30 hours per week. Services will include primary care for common and accute illnesses, psycho-social problems and preventive services.

Clinic mission:
The goal of the school-based health care clinic is to achieve the best possible physical, intellectual and emotional health of area youth by providing services which are of high quality, easy accessibility and are readily acceptable to all students.
What can the clinic do?

Care for minor acute illnesses or injuries

Coordinated care with your physician for chronic illnesses such as asthma, diabetes, ADHD or allergies

Sports physicals

Referrals to other community resources

Provide immunizations

Assistance with mental health issues

Smoking cessation and weight management

Assist family with MI Child/Medicaid/Healthy Kids application process

Testing and counseling
Will there be a charge for visits?
Charges for services are based on a sliding fee scale. Services may be billed to health insurances. No one will be refused services if they cannot pay.
How can students participate in the school based clinic?
Students must have the consent form signed by their parents to receive care, if they are under the age of 18. No birth control pills or devices are available through the clinic. No abortion counseling or referrals are available.
How will the clinic staff work with your primary care providers?
With parent permission, the staff will call and consult with your current health care providers. The staff will always encourage students to have their own primary care providers provide their care. The staff will support a student's current health care plan and augment the plan as directed by the student's doctor.

Services provided through the clinic are determined and evaluated by the Michigan Department of Community Health, the Adolescent Advisory Committee and your School Board.
