Reading the informational material now, and printing out and filling in the registration forms, will save you time when you come to a flu clinic for your vaccination! (This is probably not necessary if you are coming to the Health Department for your vaccination.)
Please bring your Medicare, Medicaid or private insurance card and any supplemental insurance cards with you. If you don't have Medicare, Medicaid or private insurance the fees are as stated above.
